Reversible Wenckebach type atrioventricular block associated with severe coronary artery disease.

چکیده
HIGH-grade atrioventricular heart block in patients with coronary artery disease is generally thought to be irreversible and in most cases atropine produces little or no effect.' Nevertheless, the possibility of significant vagal action should always be considered in any patient with heart block. When such a factor is present, the administration of atropine will produce a striking improvement in atrioventricular conduction. We are presenting detailed observations on an unusual patient with heart block: chronic atrioventricular block of the Wenckebach type was associated with severe coronary artery disease and congestive heart failure; atropine abolished the block, prevented Stokes-Adams attacks, and controlled an increase in the block that was produced by digitalis. These observations indicate that there may be functional as well as organic components in this type of heart block, and emphasize the importance of testing the ef. fects of atropine in such patients.

Second-degree Atrioventricular (Mobitz 1) Heart Block in a 52-hour-old Newborn: A Very Rare Case Report
Background: Cardiac conduction disorders are rare syndromes in neonates and children. According to the literature atrial septal defect, especially ostium secundum type, is associated with atrioventricular (AV) block. Wenckebach conduction heart block is very rare in a neonate; however, there is a dearth of research on the prevalence of this type of heart block regarding the neonates and pediatr...
متن کاملAssociation of sinus node dysfunction, atrioventricular node conduction abnormality and ventricular arrhythmia in patients with Kawasaki disease and coronary involvement.
BACKGROUND This study was performed to investigate the incidence of arrhythmias in patients with Kawasaki disease (KD). METHODS AND RESULTS Electrophysiologic studies (EPS) were performed in 40 patients (mean age: 10.3+/-5.1 years; 30 males, 10 females) with KD who had severe to moderate coronary artery disease. Clinical arrhythmias were documented in 4 patients (premature ventricular contrac...
متن کاملReversibility of Atrioventricular Block According to Coronary Artery Disease: Results of a Retrospective Study
BACKGROUND AND OBJECTIVES The causal relationship of clinically-significant atrioventricular block (AVB) and coronary artery disease (CAD) is uncertain. We investigated whether CAD is related to irreversible AVB that requires treatment with a permanent pacemaker. SUBJECTS AND METHODS We included 188 consecutive patients with new-onset AVB considering pacemaker, who had undergone invasive or n...
متن کاملWoldemar Mobitz and His 1924 classification of second-degree atrioventricular block.
Woldemar Mobitz, an early 20th century German internist, analyzed arrhythmias by graphing the relationship of changing atrial rates and premature beats to AV conduction. Through an astute mathematical approach, he was able to classify second-degree atrioventricular block into 2 types, subsequently referred to as Mobitz type I (Wenckebach) and Mobitz type II (Hay). Type I AV block was most often...
متن کاملUnsuspected coronary artery disease as cause of chronic atrioventricular block in middle age.
Attention has recently been drawn to the relatively poor prognosis of middle aged patients paced for chronic atrioventricular block when age-linked expectation of life is taken into account, and it has been suggested that this may be the result of underlying coronary artery disease, despite the absence of symptoms to suggest this. It was the purpose of this study to determine the incidence of u...
